Barbituric spiro systems are an intriguing class of heterocyclic compounds that fuse the barbituric acid core with additional cyclic frameworks via a shared spiro center. These architectures combine the rich chemical history of barbiturates with the structural complexity of spiro cyclic scaffolds, resulting in unique physicochemical properties and versatile applications. This paper explores the synthesis, structural features, pharmacological significance, and emerging roles of barbituric spiro systems in medicinal chemistry, materials science, and molecular design. We highlight recent research demonstrating how spiro barbiturates can serve as scaffolds for biologically active molecules, potential drug candidates, and functional materials, concluding with future directions and challenges.
